Terms of Service

1. Acceptance of Terms

By accessing or using the website at trueroots.la, scheduling an appointment, purchasing services, enrolling in a membership, or otherwise engaging with True Roots — Performance & Aesthetics (“True Roots,” “we,” “us,” or “our”), you agree to be bound by these Terms of Service (“Terms”). If you do not agree, do not use our services or website.

These Terms constitute a legally binding agreement between you and True Roots. By booking an appointment or receiving treatment, you confirm that you have read, understood, and agree to these Terms.

2. Description of Services

True Roots is a physician-led medical spa and performance aesthetics practice located in La Cañada Flintridge, California. We provide medical aesthetic, functional medicine, and wellness services, including but not limited to:

• Botox and neurotoxin injections
• Laser hair removal (Lumenis Splendor X)
• Hair restoration (Lumenis Folix)
• Body sculpting
• IV therapy (NAD+, Myers' Cocktail, amino acids, Vitamin C, Glutathione)
• Testosterone replacement therapy (TRT) and hormone optimization
• Peptide therapy
• Medical weight loss (GLP-1 medications)
• Functional medicine consultations

All medical services are provided under the supervision of a licensed physician. True Roots is not a hospital, urgent care, or emergency medical facility. Our services are elective in nature and are not a substitute for emergency medical care.

3. Eligibility

You must be at least 18 years of age to receive services at True Roots. Certain services may require a medical consultation and physician clearance prior to treatment. By booking an appointment, you represent that:

• You are at least 18 years old
• You are legally capable of entering into a binding agreement
• All information you provide is accurate and complete
• You will disclose all relevant medical history, medications, allergies, and health conditions

True Roots reserves the right to decline or discontinue treatment at any time if, in the physician's clinical judgment, treatment is not appropriate.

4. Consultations and Informed Consent

Certain services require an initial consultation before treatment can be provided. The initial consultation fee is $250 and applies as a credit toward your first treatment if you proceed.

Before receiving any medical treatment, you will be required to sign a separate informed consent form specific to the procedure. Informed consent forms will describe the nature of the treatment, expected outcomes, potential risks and side effects, alternative options, and aftercare requirements.

You acknowledge that no guarantee of specific results has been made. Individual outcomes vary based on factors including health status, adherence to pre- and post-treatment instructions, and individual physiology.

5. Booking, Cancellation, and No-Show Policy

Scheduling

Appointments may be scheduled online, by phone at (818) 578-4718, or by email at info@truerootsla.com.

Cancellation

We require at least 24 hours' notice for appointment cancellations or rescheduling. Cancellations made with less than 24 hours' notice may be subject to a cancellation fee of up to the full cost of the scheduled service.

No-Shows

Patients who fail to appear for a scheduled appointment without prior notice (“no-shows”) may be charged a no-show fee of up to the full cost of the scheduled service. Repeated no-shows (three or more) may result in the requirement of a prepayment deposit for future appointments.

Late Arrivals

If you arrive more than 15 minutes late, we may need to reschedule your appointment to avoid impacting other patients. This may be treated as a late cancellation at our discretion.

6. Membership Terms

True Roots offers membership tiers that provide discounts and included services. By enrolling in a membership, you agree to the following:

6.1 FOUNDATION Membership

• Monthly fee: $95/month
• Minimum commitment: 3 months
• Benefits include discounted pricing on services, Botox at member pricing ($11/unit), priority booking, and additional discounts as described at the time of enrollment

6.2 PERFORMANCE Membership

• Monthly fee: $450/month
• Minimum commitment: 6 months
• Benefits include a Treatment Track (selected at enrollment), quarterly comprehensive labs and physician check-in, Botox at member pricing, concierge booking, and additional discounts as described at the time of enrollment

6.3 General Membership Terms

• Billing. Membership fees are billed monthly to the payment method on file. You authorize True Roots to charge your payment method on a recurring basis.
• Minimum Commitment. You agree to maintain your membership for the minimum commitment period applicable to your tier. Cancellation prior to the end of the minimum commitment period will result in an early termination fee equal to the remaining months of the commitment period.
• Cancellation After Minimum Period. After the minimum commitment period, you may cancel your membership with 30 days' written notice. Cancellation requests must be submitted by email to info@truerootsla.com or in writing at our office.
• Unused Benefits. Included monthly services or credits do not roll over to subsequent months unless otherwise specified in writing.
• Modifications. True Roots reserves the right to modify membership pricing, benefits, or tier structures with 30 days' written notice to active members. Members may cancel without penalty within 30 days of receiving notice of material changes.
• Membership is Non-Transferable. Memberships are personal and may not be transferred, shared, or assigned to another individual.

7. Payment Terms

Payment Methods

We accept major credit cards, debit cards, and other payment methods as posted. Payment is due at the time of service unless otherwise arranged.

Pricing

Prices for services are as listed on our website and in our office. Prices are subject to change without prior notice, except as limited by active membership agreements.

Package Purchases

Prepaid treatment packages are non-refundable after the first session has been administered. Unused package sessions expire 12 months from the date of purchase unless otherwise stated.

Insurance

True Roots does not accept insurance. All services are self-pay. We may provide documentation for you to submit to your insurance carrier for potential reimbursement, but we make no representation that services will be covered.

Returned Payments

A fee of $35 will be charged for any returned payment or declined transaction on a recurring membership charge.

8. Medical Disclaimers

Results. Individual results vary. The information provided on our website, social media, or marketing materials is for informational purposes only and does not constitute a guarantee of specific outcomes. Before-and-after images represent individual results and may not reflect the experience of all patients.

Not Emergency Care. True Roots is not an emergency medical facility. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room.

Professional Advice. Content on our website is not intended as a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of your physician or other qualified health provider with questions about a medical condition.

Off-Label Use. Certain treatments may involve off-label use of FDA-approved medications or devices. Your treating physician will discuss this with you as part of the informed consent process.

9. Intellectual Property

All content on the True Roots website — including text, images, graphics, logos, videos, and design elements — is the property of True Roots or its licensors and is protected by copyright, trademark, and other intellectual property laws.

You may not reproduce, distribute, modify, create derivative works from, or publicly display any content from our website without prior written permission. The True Roots name, logo, and all related marks are trademarks of True Roots.

10. User Conduct

When using our website or interacting with our practice, you agree not to:

• Provide false or misleading information
• Use the website for any unlawful purpose
• Interfere with the operation of the website or our services
• Attempt to gain unauthorized access to our systems
• Scrape, harvest, or collect information from the website using automated means
• Post or transmit any defamatory, harassing, or offensive content

11. Limitation of Liability

To the maximum extent permitted by applicable law, True Roots, its owners, officers, employees, agents, and affiliated physicians shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of our services or website.

Our total liability for any claim arising from services provided shall not exceed the amount you paid for the specific service giving rise to the claim.

This limitation of liability does not apply to claims arising from gross negligence, willful misconduct, or medical malpractice to the extent such limitation is prohibited by California law.

12. Indemnification

You agree to indemnify, defend, and hold harmless True Roots, its owners, officers, employees, and agents from and against any and all claims, liabilities, damages, losses, and expenses (including reasonable attorneys' fees) arising out of or in connection with: (a) your violation of these Terms; (b) your provision of false or inaccurate information; or (c) your use of our services or website.

13. Dispute Resolution

Any dispute arising out of or relating to these Terms or the services provided by True Roots shall be governed by the laws of the State of California. You agree that any legal action shall be brought exclusively in the state or federal courts located in Los Angeles County, California.

14. Governing Law

These Terms shall be governed by and construed in accordance with the laws of the State of California, without regard to its conflict of law provisions.

15. Modifications to These Terms

True Roots reserves the right to modify these Terms at any time. Updated Terms will be posted on our website with a revised “Last Updated” date. Your continued use of our services or website after changes are posted constitutes acceptance of the modified Terms.

Material changes affecting active membership agreements will be communicated to members by email at least 30 days in advance.

16. Severability

If any provision of these Terms is found to be unenforceable or invalid, that provision shall be limited or eliminated to the minimum extent necessary, and the remaining provisions shall remain in full force and effect.

17. Entire Agreement

These Terms, together with our Privacy Policy, any signed informed consent forms, and any membership agreement, constitute the entire agreement between you and True Roots regarding the subject matter herein.
